Job Description

Providing skilled nursing care in a hospital stepdown unit, this role focuses on delivering patient-centered care aligned with professional nursing standards. Reporting to the Nurse Manager or Director, the Clinical Nurse coordinates and implements comprehensive nursing processes for patients requiring intermediate-level care.

Responsibilities include continuous patient assessment, development and revision of individualized care plans, and collaboration with the interdisciplinary healthcare team to ensure seamless care delivery. The nurse will actively participate in educational programs to enhance professional growth and utilize patient education materials to support patient and family understanding. Leadership duties involve contributing to unit goals, promoting cost-effective care, and maintaining a safe, clean environment.

Key Duties:

  • Conduct thorough assessments integrating physical, psychosocial, educational, and safety needs.
  • Evaluate patient outcomes and adjust care plans with guidance.
  • Perform bedside point-of-care testing and prioritize care effectively.
  • Collaborate with patients, families, and multidisciplinary teams in care planning.
  • Participate in performance improvement activities and regulatory compliance.
