Job Description

This role involves providing direct nursing care in a step-down unit catering to medically complex patients awaiting community placement. It requires a Registered Nurse adept at assessing, planning, and delivering patient-centered care while collaborating closely with patients, families, and the healthcare team. Candidates must be capable of recognizing subtle changes in patient conditions and managing care in a setting without immediate ICU escalation.

Key Responsibilities:

  • Deliver direct patient care using the nursing process, including assessment, planning, implementation, evaluation, and documentation.
  • Administer medications and treatments according to physician orders and established protocols.
  • Collaborate with multidisciplinary teams to coordinate comprehensive patient care.
  • Monitor and identify patient status changes, including de-escalating or responding to deteriorations.
  • Provide supervision to Licensed Practical Nurses (LPNs) and assistive personnel.
  • Address patient and family education and discharge planning needs throughout care episodes.
  • Float between departments and nearby locations within a 20-mile radius as needed.
  • Maintain readiness for weekend shifts and on-call or call-back duties.

Qualifications and Desired Experience:

  • Current Registered Nurse (RN) license.
  • Successful completion of a Basic Cardiac Arrhythmia (BCA) test onsite is mandatory, with one opportunity to retest if necessary.
  • Experience in step-down or medical-surgical nursing preferred.
  • Ability to manage medically complex patients outside acute care settings.
  • Strong clinical judgment and patient assessment skills; charge nurse experience is a plus.
