Job Description

Provide skilled nursing care in a Long Term Care (LTC) setting located on a hospital campus. This role involves performing assessments, treatments, medication administration, and coordinating with medical providers to ensure optimal patient outcomes. Responsibilities include admitting and discharging patients, assisting with activities of daily living (ADLs), and specialized care such as gastrostomy tube management, urinary catheter care, nebulized treatments, wound vac therapy, CPAP maintenance, and tracheostomy care.

Key Responsibilities:

  • Conduct comprehensive patient assessments and implement tailored nursing care plans
  • Administer medications accurately and monitor patient responses
  • Communicate pertinent patient information promptly to physicians or nurse practitioners
  • Manage admissions and discharges efficiently
  • Provide assistance with ADLs and deliver specialized clinical care as required
  • Maintain compliance with facility policies and state regulations

Qualifications:

  • Current active Registered Nurse (RN) license with endorsement or ability to obtain licensure in the State of Hawaii prior to assignment start
  • Experience in rehabilitation and long-term care nursing preferred
  • Proven ability to manage complex nursing tasks including wound and respiratory care
  • Flexibility to work varied 8-hour shifts, including weekends and holidays
  • Commitment to timely arrival for employee health appointment the Thursday before start date
