This role focuses on acute rehab and post-acute care with a 24-bed facility, supporting patients in their recovery journey. The position offers a balanced patient ratio of 1:6-8, enabling personalized and attentive care.

Key Responsibilities:
- Deliver comprehensive nursing care to patients in an acute rehab/post-acute setting
- Monitor patient progress and collaborate with interdisciplinary teams to adjust care plans
- Administer medications and treatments as prescribed, adhering to facility protocols
- Educate patients and families on care plans and rehabilitation processes
- Maintain accurate and timely documentation in accordance with regulatory standards
Qualifications:
- Valid RN licensure in the state of Indiana
- Experience in Rehabilitation Nursing and Long-Term Care settings
- Strong clinical assessment and patient management skills
- Effective communication and teamwork abilities
