A rewarding opportunity awaits for a Registered Nurse skilled in Rehabilitation and Long-Term Care to provide compassionate care to patients in nursing home and assisted living settings. This contract position, based in Clermont, FL, focuses on delivering comprehensive care designed to treat patients in place when possible, thereby minimizing unnecessary hospitalizations and facilitating successful transitions to home care.
Key Responsibilities:
- Collaborate closely with Nurse Practitioners and interdisciplinary care teams to coordinate patient care.
- Conduct routine monthly health assessments and additional visits as needed to monitor patient progress.
- Participate in meetings and communicate updates to support individualized care planning.
- Coordinate care activities within skilled nursing facilities to address resident needs effectively.
- Assist with clinical and quality improvement tasks, including identifying and addressing gaps in care.
- Engage regularly with residents, families, nursing staff, and care teams to ensure a seamless healthcare experience.
- Conduct resident rounds and support Advanced Care Planning.
Qualifications:
- ASN or BSN degree.
- Valid Florida RN license.
- Current Basic Life Support (BLS) certification.
- At least one year of experience in Long-Term Care, Care Coordination, or Care Management preferred.
Schedule/Hours:
- Monday through Friday, 8 AM to 5 PM.
- No weekends or holidays required.
