Job Description
Deliver high-quality patient care within nursing home and assisted living settings as part of an innovative program designed to support insurance members in skilled facilities. This contract position specializes in treating patients in place to prevent unnecessary hospitalizations and fostering successful transitions back to the home environment. The role emphasizes monthly health assessments with more frequent visits as needed to provide comprehensive care, resources, and support for patients, ultimately aiming to reduce reliance on emergency room visits.
Schedule:
- Monday through Friday, 8:00 AM to 5:00 PM
- No weekends or holidays required
Key Responsibilities:
- Conduct rounds on patients at multiple facilities
- Complete routine monthly visits as delegated by the Nurse Practitioner
- Review residents' status and support ongoing progress toward healthcare goals
- Attend meetings on behalf of the Nurse Practitioner and provide updates to the Interdisciplinary Care Team
- Perform essential care coordination activities to ensure continuity and quality of care
Qualifications:
- Registered Nurse with ASN or BSN credentials
- Valid Basic Life Support (BLS) certification
- Active RN license issued by the state of New York
- Experience in long term care or assisted living environments is preferred
Compensation: This position offers a competitive hourly rate ranging from $65 to $80.
