Job Description

Provide high-quality nursing care for patients residing in skilled nursing and assisted living environments. This contract position is centered around comprehensive support for insured members, with a mission to minimize unnecessary hospitalizations and foster smooth transitions back to home settings. You'll be instrumental in providing monthly (or more frequent as needed) health assessments, focusing on improving patient outcomes, coordinating care, and promoting effective treatment on-site.

Schedule:

  • Monday Friday, 8 AM 5 PM
  • No weekends or holidays

Qualifications:

  • Current RN license valid in New York State
  • Bachelors or Associates degree in Nursing (BSN or ASN)
  • Basic Life Support (BLS) certification
  • Experience or strong interest in long-term care settings

Core Responsibilities:

  • Conduct regular rounds and monthly assessments of residents across multiple facilities
  • Carry out delegated routine visits as assigned by the Nurse Practitioner
  • Continually review and monitor resident status, supporting their progress toward established care goals
  • Attend care meetings in place of the Nurse Practitioner, reporting key updates to the interdisciplinary care team
  • Facilitate and communicate care coordination activities to ensure continuity of care and resource provision
  • Focus on preventing avoidable ER visits and hospital admissions through proactive, in-place treatments and interventions

Compensation:

  • Competitive hourly rate ranging from $65 to $80, reflecting experience and qualifications
