Take on a vital role providing meaningful clinical support and care coordination to residents in skilled nursing and assisted living settings. This contract opportunity centers on delivering comprehensive, proactive healthcare to insurance members, with a strong emphasis on treat in place to reduce unnecessary hospitalizations and support positive transitions back home.
Schedule:
- Monday to Friday, 8 AM 5 PM
- No weekends or holidays
Required Qualifications:
- Active RN license in New York
- Associates or Bachelors Degree in Nursing (ASN or BSN)
- Valid BLS certification
- Previous nursing experience in long-term care or related settings preferred
Key Responsibilities:
- Conduct regular patient rounds at multiple facilities, focusing on nursing home and assisted living populations
- Perform monthly health assessments, with increased frequency as clinically indicated
- Complete routine monthly visits as assigned by the Nurse Practitioner
- Monitor, review, and report on residents status, supporting patients in achieving their care goals
- Serve as a clinical liaison by attending meetings on behalf of the Nurse Practitioner and providing comprehensive updates to interdisciplinary teams
- Manage and coordinate care activities, helping optimize healthcare resources to reduce avoidable emergency department visits and hospital admissions
This position offers highly competitive compensation ranging from $65 to $80 per hour, reflecting the essential contributions of skilled registered nurses in long-term care environments.
