Job Description

This contract position emphasizes comprehensive care for insurance members in skilled nursing and assisted living facilities, aiming to provide treatment in place when appropriate. Key Responsibilities:
- Conduct routine rounds and monthly health assessments on patients across multiple facilities
- Complete delegated monthly visits as assigned by the Nurse Practitioner (NP)
- Monitor and review residents status, assisting in achieving their health goals
- Represent the NP during meetings, ensuring clear communication with the Interdisciplinary Care Plan (ICP) and Interdisciplinary Care Team (ICT)
- Perform care coordination to enhance patient outcomes and resource access
Qualifications/Desired Experience:
- Registered Nurse (RN) with a BSN or ASN degree
- Valid New York RN License
- Current Basic Life Support (BLS) certification
- Experience in long term care or skilled nursing settings preferred
Schedule/Hours:
- Monday through Friday
- 8 AM to 5 PM
- No weekends or holidays
