Job Description
Take on an impactful nursing assignment dedicated to supporting patients in nursing home and assisted living settings. This contract role involves providing comprehensive care for insurance members residing in skilled nursing and assisted living facilities, with an emphasis on delivering treatment onsite whenever appropriate. The primary objective is to avoid unnecessary hospitalizations and facilitate a smooth transition back to a home environment, enhancing overall patient outcomes.
Schedule:
- Monday through Friday, 8:00 AM to 5:00 PM
- No required weekend or holiday shifts
Key Responsibilities:
- Conduct regular rounds across multiple facilities, ensuring consistent patient oversight
- Complete routine monthly assessments for residents, as delegated by the Nurse Practitioner (NP), with additional follow-ups as needed
- Review each residents health status, collaborating with the care team to help achieve individual progress toward health goals
- Represent the NP at meetings, ensuring timely and clear communication of updates to the Interdisciplinary Care Team (ICT) and the care plan (ICP)
- Perform care coordination activities to optimize resources, prevent unnecessary emergency department visits or hospitalizations, and support residents in transitioning to home care wherever possible
Qualifications:
- Active Registered Nurse (RN) license in New York
- Bachelors or Associates Degree in Nursing (BSN or ASN)
- Current Basic Life Support (BLS) certification
- Prior experience in long-term care, skilled nursing, or similar environments preferred
