Deliver high-quality nursing care to individuals in long-term care and assisted living environments as part of a collaborative, patient-focused program. This opportunity centers on providing comprehensive, ongoing care for insurance members within nursing homes and assisted living facilities, aiming to reduce unnecessary hospitalizations and encourage positive transitions to home-based care whenever appropriate.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
• No weekend or holiday shifts required

Key Responsibilities:

• Conduct patient rounds in multiple facilities, ensuring ongoing assessments and timely follow-up
• Perform monthly health assessments and routine visits as delegated by the Nurse Practitioner
• Evaluate residents progress, assist in meeting care goals, and document updates accordingly
• Attend meetings on behalf of the Nurse Practitioner, sharing resident information and care updates with care teams
• Carry out care coordination activities, including resource allocation and communication between interdisciplinary teams
• Focus on avoiding unnecessary emergency room visits or hospital admissions by delivering proactive, in-place care solutions

Qualifications:

• Active Registered Nurse (RN) license in New York
• Associates or Bachelors degree in Nursing (ASN or BSN)
• Current Basic Life Support (BLS) certification
• Experience or interest in long-term care or assisted living settings preferred
