Bring your passion for patient-centered care to a dynamic setting focused on supporting skilled nursing and assisted living residents. This contract role is dedicated to improving health outcomes for insurance members by providing regular, comprehensive health assessments and supporting timely treatment interventions within the facilityhelping to prevent unnecessary hospital visits and ensuring smoother transitions to home care.
Schedule:
- Monday Friday, 8 AM 5 PM
- No weekends or holidays required
Responsibilities:
- Conduct patient rounds in multiple nursing home and assisted living settings
- Complete routine monthly visits as delegated by the Nurse Practitioner, with increased frequency as patient needs dictate
- Monitor and review residents conditions, supporting their progress toward established care goals
- Represent the Nurse Practitioner at facility meetings, relaying essential updates to the Interdisciplinary Care Team (ICT)
- Carry out essential care coordination activities to facilitate high-quality, integrated care
Required Qualifications:
- Valid RN licensure in New York (NY)
- Bachelors or Associates degree in Nursing (BSN or ASN)
- Current Basic Life Support (BLS) certification
- Experience in long-term care or similar healthcare environments is preferred
