Job Description
A nursing professional focused on long-term care and delivering comprehensive support to patients in skilled nursing and assisted living environments is needed for a contract position based in Brooklyn, NY. This role centers on providing ongoing health assessments and care coordination with the goal of avoiding unnecessary hospitalizations and promoting successful transitions back to home settings.
Schedule:
- Monday to Friday
- 8 AM to 5 PM
- No weekends or holidays
Key Responsibilities:
- Perform patient rounds across multiple nursing home and assisted living facilities
- Complete routine monthly visits as delegated by the Nurse Practitioner (NP)
- Review residents status and support progress toward individualized care goals
- Represent the NP in interdisciplinary team meetings and communicate updates to the interdisciplinary care plans (ICP) and teams (ICT)
- Execute care coordination activities to manage patient needs effectively
Qualifications:
- Registered Nurse (RN) with either a Bachelor of Science in Nursing (BSN) or an Associate Degree in Nursing (ASN)
- Valid New York State RN license
- Basic Life Support (BLS) certification
- Experience in long-term care nursing is highly desirable
