Job Description
Seeking an experienced Registered Nurse to provide care in nursing home and assisted living settings. This role is essential to delivering comprehensive healthcare for insurance members residing in skilled nursing or assisted living facilities. The primary goal is to treat residents in place whenever feasible, aiming to reduce unnecessary hospital visits and foster successful transitions back home. Youll conduct regular health assessments, ensuring that residents needs are addressed promptly and effectively.
Schedule:
- Monday Friday, 8 AM 5 PM
- No weekends or holidays
Key Responsibilities:
- Complete rounds on patients located in multiple facilities.
- Perform routine monthly visits as delegated by the Nurse Practitioner.
- Review resident statuses and assist them in achieving progress toward personalized care goals.
- Attend meetings on behalf of the Nurse Practitioner, providing thorough updates to the Interdisciplinary Care Plan (ICP) within the Interdisciplinary Care Team (ICT).
- Coordinate care and facilitate communication among healthcare providers to promote optimal outcomes.
- Support efforts to minimize emergency room use and unnecessary hospitalizations by providing preventive care and appropriate interventions.
Qualifications:
- Current RN license in New York
- Bachelors or Associates degree in Nursing (BSN or ASN)
- Valid Basic Life Support (BLS) certification
- Previous experience in long-term care or similar healthcare environment preferred
