Job Description

Step into a vital clinical role supporting residents within nursing home and assisted living environments, providing comprehensive care for insurance members to help avoid unnecessary hospitalizations and successfully transition patients to their homes. This contract opportunity focuses on improving healthcare outcomes through regular health assessments, resource coordination, and direct patient engagementideal for registered nurses passionate about long-term care.

Schedule:

  • Monday through Friday, 8:00 AM 5:00 PM
  • No weekend or holiday shifts

Key Responsibilities:

  • Conduct patient rounds across multiple skilled nursing and assisted living facilities
  • Perform routine monthly health assessments, and additional visits as indicated, following delegation from the Nurse Practitioner
  • Review residents health status and support progress toward individualized care goals
  • Represent the Nurse Practitioner in relevant facility meetings, keeping the Interdisciplinary Care Team (ICT) informed of updates
  • Manage care coordination activities to ensure seamless delivery of services and resource utilization
  • Contribute to proactive interventions that reduce emergency room visits and preventable hospital admissions

Qualifications:

  • Active Registered Nurse (RN) license in New York; BSN or ASN accepted
  • Current Basic Life Support (BLS) certification
  • Experience in long-term care, skilled nursing, or assisted living settings preferred
