This contract opportunity allows registered nurses to make a significant impact by delivering comprehensive care for insurance members residing in skilled nursing and assisted living facilities. The primary objective is to assess and treat patients on-site, reduce unnecessary hospitalizations, and ensure a smooth transition to the home environment.

Schedule:

• Monday through Friday, 8 AM to 5 PM
• No weekends or holidays

Key Responsibilities:

• Complete patient rounds across multiple facilities, ensuring all are visited and assessed as scheduled
• Conduct routine monthly health assessments and follow-up visits as delegated by the Nurse Practitioner (NP)
• Review residents status regularly, collaborating to help patients reach their health goals
• Represent the NP at meetings and provide timely communication and updates to the overall care team
• Perform care coordination activities, liaising effectively with interdisciplinary care teams (ICT)

Qualifications:

• Current RN license in the state of New York (NY RN License)
• Associates or Bachelors degree in Nursing (ASN or BSN)
• Basic Life Support (BLS) certification
• Experience with long-term care or similar settings preferred
