Job Description

Provide essential nursing care both virtually and in-person as a Digital Acute Care Registered Nurse, focused on adult patients within a 30-minute radius of a central location in Washington, IL. This role blends telehealth technology in a command center with direct patient care in their home environment, aiming to enhance patient quality of life through timely intervention and collaborative care.

Responsibilities:

  • Deliver virtual and hands-on nursing care using telehealth tools and home visits.
  • Monitor patient conditions proactively to prevent complications and provide early interventions.
  • Collaborate closely with multidisciplinary teams, patients, and families to ensure coordinated, high-quality care.
  • Utilize evidence-based clinical decision support to guide nursing practices.
  • Serve as a patient advocate and connect patients with appropriate healthcare and community resources.
  • Maintain accurate documentation using electronic medical records (EMR) and communication platforms.
  • Comply with motor vehicle regulations and maintain valid driving credentials while traveling to patient homes.
  • Demonstrate accountability for cost-effective and efficient care delivery.

Qualifications and Experience:

  • Associate Degree in Nursing required; Bachelors Degree preferred.
  • Current Registered Nurse (RN) license.
  • American Heart Association Basic Life Support (BLS) certification.
  • At least 1 year of recent RN experience with adult acute care patients.
  • Minimum 2 years experience collaborating and communicating effectively with healthcare professionals.
  • Valid drivers license and acceptable Motor Vehicle Report.
  • Proficient computer skills, including Microsoft Office and electronic medical records.
  • Strong interpersonal, communication, analytical, and problem-solving abilities.
