Job Description

Providing comprehensive virtual and in-home nursing care, this role is integral in delivering acute care services to adult patients through a blend of telehealth technology and direct patient interaction. Operating within a 30-minute radius from a central location, the nurse will use a provided fleet vehicle to travel to patient homes, ensuring timely and effective healthcare delivery.

Responsibilities:

  • Deliver virtual and in-person nursing care to adult patients using telehealth and home visits
  • Monitor patient health proactively, focusing on prevention and early intervention
  • Utilize evidence-based practices and clinical decision support tools to provide quality care
  • Collaborate closely with patients, families, and healthcare team members to ensure coordinated care
  • Act as a patient advocate and resource for healthcare providers and community services
  • Maintain accurate documentation and communication through electronic medical records

Qualifications:

  • Associate Degree in Nursing required; Bachelors Degree preferred
  • Valid RN license and current American Heart Association BLS certification
  • Minimum of 1 year recent RN experience in adult acute care
  • At least 2 years experience collaborating with healthcare professionals effectively
  • Strong communication, interpersonal, analytical, and computer skills, including Microsoft Office proficiency
  • Valid drivers license, with ability to provide required motor vehicle report and insurance documentation
  • Experience with electronic medical records and communication platforms preferred
