Job Description

Deliver comprehensive nursing care both virtually and in patients homes as a Digital Acute Care Registered Nurse. This role integrates telehealth technology and in-person visits to support adult patients in managing acute care needs while optimizing their quality of life at home. Engage with a multidisciplinary team to provide proactive monitoring, early interventions, and ongoing patient advocacy across a diverse patient population.

Location: This position is based in Pekin, IL, with travel required to patient homes within a 30-minute radius of the central office. A fleet vehicle will be provided to facilitate patient visits, enhancing your ability to deliver timely and efficient care in the community.

Responsibilities:

  • Provide acute care nursing using telehealth technology and direct in-home care.
  • Monitor and assess patient health status, offering early interventions as needed.
  • Collaborate effectively with healthcare professionals, patients, and families to ensure high-quality patient-centered care.
  • Utilize clinical decision support and evidence-based practices to guide nursing care.
  • Act as a patient advocate and professional resource to both healthcare providers and community resources.
  • Maintain documentation and communicate regularly within the care team.

Qualifications:

  • Associate Degree in Nursing required; Bachelors Degree preferred.
  • Current RN licensure with at least 1 year of recent experience in adult acute care.
  • Minimum 2 years demonstrating strong communication and collaboration in healthcare settings.
  • Valid state drivers license and compliance with motor vehicle and insurance requirements.
  • Current American Heart Association BLS certification.
  • Proficient computer skills, including Microsoft Office and electronic medical records (EMR).
  • Detail-oriented with strong analytical and problem-solving abilities.
  • Excellent interpersonal and communication skills.
