Responsibilities:

  • Deliver nursing care through telehealth technology in a centralized command center and via direct in-home visits.

  • Serve as a patient advocate and liaise with various healthcare and community resources.

Required Qualifications:

  • Associate Degree in Nursing.
  • Current RN licensure.
  • American Heart Association BLS certification.
  • Valid drivers license with Motor Vehicle Report and insurance coverage.

  • At least 2 years in a role demanding effective communication and collaboration with health professionals.
  • Strong interpersonal, communication, analytical, and computer skills, including proficiency with Microsoft software.

Preferred Qualifications:

  • Bachelors Degree in Nursing.
  • Experience with electronic medical records (EMR) and communication platforms.
  • Reliable transportation skills for patient home visits.
