Job Description

A registered nurse with a passion for innovative patient care is needed to deliver both virtual and in-person nursing services. This role combines telehealth support from a centralized command center with hands-on care in patients homes, providing a unique opportunity to impact adult patients quality of life through technology and personalized attention. This position involves travel within a 30-minute radius using a provided fleet vehicle, supporting a proactive approach to acute care in the community.

Responsibilities include:

  • Monitoring and managing patient health through telehealth and in-home visits
  • Early intervention and ongoing patient assessment to prevent escalation
  • Collaborating with healthcare teams, patients, and families to ensure quality care
  • Advocating for patient needs and connecting with community resources
  • Documenting care accurately using electronic medical records (EMR) and communication platforms

Qualifications:

  • Associates Degree in Nursing required; Bachelors Degree preferred
  • Current RN license and American Heart Association BLS certification
  • Minimum 1 year recent experience in adult acute care nursing
  • At least 2 years in roles requiring effective communication and collaboration with healthcare providers
  • Valid state drivers license with acceptable motor vehicle report and current auto insurance coverage
  • Strong interpersonal, analytical, and computer skills, including proficiency with Microsoft Office
  • Comfortable with travel and hands-on care in patients homes within a 30-minute radius
