Job Description

Provide advanced nursing care through a combination of virtual monitoring and in-person visits to adult patients within a 30-minute radius of the designated OnCall building. This role involves utilizing telehealth technology from a centralized command center as well as delivering hands-on acute care in patients homes. The goal is to enhance patient quality of life by offering timely interventions, preventive care, and ongoing support in collaboration with multidisciplinary teams.

Key Responsibilities:

  • Deliver skilled nursing care virtually and onsite for a diverse adult acute care population
  • Monitor patient status proactively using evidence-based practices and clinical decision support tools
  • Communicate effectively with patients, families, and healthcare providers to promote optimal outcomes
  • Advocate for patient needs and act as a resource within the care network
  • Maintain accurate documentation and utilize electronic medical records proficiently
  • Travel safely to patient homes within a 30-minute radius using a provided fleet vehicle

Qualifications:

  • Associate degree in Nursing required; Bachelors degree preferred
  • Current Registered Nurse (RN) license
  • American Heart Association BLS certification
  • Valid drivers license with a clean Motor Vehicle Report and proof of insurance
  • Minimum 1 year recent experience in adult acute care nursing
  • At least 2 years in roles requiring collaboration with healthcare professionals
  • Strong interpersonal, communication, analytical, and computer skills (Microsoft Office proficiency)
  • Familiarity with electronic health records and telehealth platforms preferred
