Providing compassionate, skilled nursing care in patients homes, this role offers an opportunity to impact lives meaningfully while working independently within a flexible schedule. As a Home Health RN, you will deliver personalized, patient-centered care across a wide range of clinical needs.

Location:
Based in Yuma, AZ, this position involves daily travel within an assigned territory to patient homes. Yuma offers a warm, desert climate with a strong sense of community and access to outdoor activities like hiking and local cultural events, creating a balanced lifestyle outside of work.

Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, identify changes in condition, and communicate promptly with healthcare teams and physicians.
• Educate patients and families on health conditions, treatments, and preventive care.
• Maintain accurate documentation using Home Care Home Base (HCHB) electronic system.
• Collaborate closely with therapists, social workers, and home health aides to ensure coordinated care.
• Participate in case conferences, staff meetings, and continuing education to uphold clinical excellence.
• Adhere to all regulatory and agency standards for patient care and documentation.

Qualifications:

• Active RN license in good standing for state practice.
• Minimum 1 year of recent Home Health nursing experience with strong clinical assessment skills.
• Proficiency in Home Care Home Base (HCHB) and OASIS assessment tool required.
• Reliable transportation, valid drivers license, and auto insurance necessary.
• Ability to work independently with effective time management and communication skills.

Preferred:

• BSN degree.
• Experience managing 2532 patient care points weekly.
• Knowledge of Medicare and home health compliance standards.
