Providing compassionate, skilled nursing care in patients homes, this role offers an opportunity to impact lives meaningfully while working independently within a flexible schedule. As a Home Health RN, you will deliver personalized, patient-centered care across a wide range of clinical needs.
Location:
Based in Yuma, AZ, this position involves daily travel within an assigned territory to patient homes. Yuma offers a warm, desert climate with a strong sense of community and access to outdoor activities like hiking and local cultural events, creating a balanced lifestyle outside of work.
Responsibilities:
- Perform comprehensive OASIS assessments and develop individualized care plans.
- Deliver skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, identify changes in condition, and communicate promptly with healthcare teams and physicians.
- Educate patients and families on health conditions, treatments, and preventive care.
- Maintain accurate documentation using Home Care Home Base (HCHB) electronic system.
- Collaborate closely with therapists, social workers, and home health aides to ensure coordinated care.
- Participate in case conferences, staff meetings, and continuing education to uphold clinical excellence.
- Adhere to all regulatory and agency standards for patient care and documentation.
Qualifications:
- Active RN license in good standing for state practice.
- Minimum 1 year of recent Home Health nursing experience with strong clinical assessment skills.
- Proficiency in Home Care Home Base (HCHB) and OASIS assessment tool required.
- Reliable transportation, valid drivers license, and auto insurance necessary.
- Ability to work independently with effective time management and communication skills.
Preferred:
- BSN degree.
- Experience managing 2532 patient care points weekly.
- Knowledge of Medicare and home health compliance standards.
