Deliver high-quality patient care directly in-home as an experienced Registered Nurse specializing in home health services. This field-based, full-time contract position offers the opportunity to work within patients homes and make a meaningful difference in their health and well-being. The assignment is for 13 weeks, with the possibility for extension.

Schedule:

• Monday to Friday, 8:00 AM 5:00 PM

Role Duties:

• Complete thorough OASIS assessments and develop tailored care plans for each patient.
• Administer skilled nursing interventions, including medication management, wound care, IV therapy, and monitoring chronic conditions.
• Perform daily travel within your assigned territory, visiting 35 patients per day and managing a caseload of approximately 30 points per week.
• Educate patients and families regarding care plans, medication, prevention, and disease management.
• Document all care accurately and efficiently using the Home Care Home Base (HCHB) electronic system.
• Collaborate with a multidisciplinary team to coordinate comprehensive patient care.
• Participate in case conferences, attend staff meetings, and engage in continuing education sessions as needed.
• Uphold regulatory and agency standards for patient care and documentation.

Qualifications:

• Active, unencumbered RN license in Florida
• At least 1 year of recent home health experience, preferably in the field
• Strong proficiency with Home Care Home Base (HCHB) and OASIS assessment tools
• Reliable car, valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
