Job Description

This full-time opportunity is ideal for a skilled RN passionate about home health and dedicated to delivering patient-centered services in a field-based setting.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

Qualifications:

  • Active RN license in good standing for the state of practice
  • Minimum of 1 year recent experience in Home Health (field experience strongly preferred)
  • Proficient with Home Care Home Base (HCHB) software
  • OASIS assessment tool experience essential
  • Reliable transportation, valid drivers license, and current auto insurance

Responsibilities:

  • Conduct thorough patient OASIS assessments and develop tailored care plans
  • Deliver skilled nursing interventions including wound care, medication administration, IV therapy, and chronic disease management
  • Monitor and evaluate patient progress, identify significant changes, and communicate effectively with care team and physicians
  • Provide education to patients and families regarding disease processes, medications, and ongoing care needs
  • Ensure accurate and timely documentation in HCHB
  • Collaborate with a multidisciplinary teamtherapists, social workers, and home health aidesto optimize patient outcomes
  • Attend case conferences, staff meetings, and participate in ongoing education
  • Uphold regulatory and agency standards in documentation and care delivery

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
