A fulfilling position awaits an experienced Registered Nurse with demonstrated home health expertise. This 13-week full-time contract role is ideal for RNs who thrive in delivering personalized care, ensuring meticulous documentation, and confidently supporting patients in their homes. You'll be a valued part of a professional and collaborative care team, providing a vital service to patients in need.
Schedule & Duration:
- 13-week contract, with extension opportunities
- Full-time, Monday through Friday
- On-call duties may be required
Key Responsibilities:
- Deliver skilled in-home nursing care and interventions
- Complete patient assessments and timely, accurate OASIS documentation
- Work in close partnership with physicians and interdisciplinary care teams to craft individual care plans
- Perform ongoing wound care, medication administration, patient education, and monitoring of chronic conditions
- Communicate status changes and ensure care coordination with the clinical team
Preferred Skills & Experience:
- Active RN license
- Minimum of one year of recent home health RN experience required
- Prior OASIS experience is essential
- Familiarity with HomeCare HomeBase software strongly preferred
- Reliable transportation and current drivers license
- Ability to independently manage a dedicated patient visit schedule
