Advance your nursing career in home health by providing compassionate, individualized patient care across the Greensboro and High Point areas. This contract role is ideal for experienced registered nurses with both home health and travel nursing backgrounds, seeking a dynamic and rewarding opportunity. The assignment is for 8 weeks, with the potential to extend to 12 weeks depending on patient census.
Schedule and Hours:
- Monday through Friday, 8:00am to 5:00pm
- Day shifts only (5x8-hour shifts per week)
Key Qualifications:
- Active and unrestricted RN license
- Minimum of one year case management experience required
- At least one year of home health nursing experience (HCHB proficiency strongly preferred)
- Previous travel nursing experience is essential
- Valid drivers license and reliable personal vehicle (mileage reimbursed at the federal rate)
- Ability to manage and prioritize a diverse patient caseload independently
Responsibilities:
- Deliver home-based nursing care, ensuring clinical excellence and regulatory compliance
- Case manage a designated patient caseload, including assessment, care planning, and follow-up
- Coordinate care across interdisciplinary teams and communicate effectively with physicians and family members
- Travel between patient residences throughout the day, providing support across the Greensboro/High Point area
- Document patient care promptly and accurately in accordance with agency protocols
