Make a meaningful impact as an experienced Registered Nurse specializing in Home Health. This opportunity offers you the chance to provide personalized, compassionate care directly in the homes of patients in the Greensboro and High Point areas. This is a contract assignment for 8 weeks, with the potential to extend to 12 weeks based on census needs.

Schedule:

• Monday through Friday, 8:00 am 5:00 pm
• Day shift; five 8-hour shifts per week

Qualifications/Experience:

• Active RN license
• Minimum of one year of case management experience required
• Home Health nursing experience (HCHB + Travel experience required)
• Experience managing a caseload independently
• Must have a reliable personal vehicle; travel between patient homes is required throughout each workday

Key Responsibilities:

• Manage and deliver comprehensive nursing care for a home health patient caseload
• Conduct assessments, develop care plans, implement physician orders, and monitor patient progress
• Maintain accurate clinical records in accordance with home health standards and regulations
• Coordinate with interdisciplinary teams to ensure holistic patient care
• Educate patients and families to promote health, safety, and improved quality of life
• Travel within assigned territory (Greensboro/High Point); mileage reimbursement provided at the federal rate
