Job Description

Deliver skilled nursing care in patients homes, providing professional assessment, planning, and supervision of nursing services in compliance with state-specific regulations. This travel nursing opportunity involves coordinating care under the direction of a Clinical Manager, supervising Home Health Aides and Licensed Practical/Vocational Nurses, and ensuring high-quality patient outcomes.

Key Responsibilities:

  • Provide home nursing care according to physician orders and agency policies.
  • Assess and evaluate patient conditions regularly, reporting changes promptly.
  • Develop, implement, review, and revise individualized plans of care.
  • Supervise Home Health Aides and LPN/LVNs according to state guidelines.
  • Maintain accurate documentation and communication in patient records.
  • Comply with state and federal home care regulations.
  • Attend periodic office meetings and maintain reliable transportation for home visits.

Qualifications & Experience:

  • Current, unencumbered RN license in the assigned state; Nurse Licensure Compact eligibility preferred.
  • Minimum 2-3 years of nursing experience.
  • Current CPR certification.
  • Ability to assess patient status with consideration for age-specific needs.
  • Proficient with computers and able to navigate online training and HR systems.
  • Graduate of an accredited nursing program; BSN preferred for Illinois assignments.
  • Familiarity with state-specific nursing regulations (Arizona, Illinois, Oregon, Wisconsin, Nevada as applicable).
