This full-time, 13-week contract role offers a dynamic MondayFriday schedule from 8:00 AM to 5:00 PM, focusing on thorough patient assessments, care plan development, and collaborative care delivery.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Role Responsibilities:

• Complete OASIS assessments and develop individualized care plans for home health patients.
• Provide skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, proactively address changes in patient status, and maintain open communication with the healthcare team.
• Educate patients and their families regarding diagnoses, medications, treatments, and preventative measures.
• Ensure meticulous and timely charting using Home Care Home Base (HCHB).
• Collaborate seamlessly with interdisciplinary team membersphysical therapists, occupational therapists, social workers, and home health aides.
• Attend case conferences, meetings, and continue professional growth through ongoing education.
• Uphold all regulatory requirements for documentation and patient care delivery.
• Travel to patients homes within the assigned territory, with mileage reimbursement provided.

Qualifications:

• Active RN license in the state of practice, in good standing.
• Minimum one year of recent home health nursing experience; field experience strongly preferred.
• Strong proficiency in Home Care Home Base (HCHB) and OASIS documentation.
• Reliable transportation, valid drivers license, and current auto insurance.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
