Take the next step in your nursing career by providing high-quality, compassionate care to patients in their own living environments. This full-time, field-based contract role offers a 13-week assignment, delivering essential home health services and supporting patient independence.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Active, unencumbered RN license in the state required
• At least 1 year of recent experience in Home Health, with direct field exposure strongly preferred
• Proven proficiency using Home Care Home Base (HCHB) electronic documentation
• Demonstrated OASIS assessment expertise
• Dependable transportation, valid drivers license, and current auto insurance

Core Responsibilities:

• Complete thorough OASIS assessments and develop tailored patient care plans
• Deliver skilled nursing interventions including wound care, IV therapy, chronic condition management, and medication administration
• Monitor patient status, identify and promptly report changes, and communicate closely with the care team
• Teach patients and families about care plans, medications, and preventive health practices
• Maintain precise and timely documentation with Home Care Home Base (HCHB)
• Collaborate with allied health professionals to deliver comprehensive, coordinated patient care
• Participate in case conferences, staff meetings, and ongoing education
• Adhere to all clinical, regulatory, and agency guidelines

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
