Your next nursing opportunity starts here. We are seeking an experienced Registered Nurse (RN) with strong OASIS documentation skills to provide exceptional home health care on a 13-week contract basis. This full-time position offers a dynamic, patient-centered environment where you can use your clinical expertise to support individuals in their own homes.
Schedule and Hours:
- Full-time, Monday through Friday
- On-call coverage may be required
- Structured daily visit expectations
Key Responsibilities:
- Deliver high-quality in-home nursing care and skilled interventions
- Perform comprehensive patient assessments and complete accurate, timely OASIS documentation
- Collaborate with physicians and interdisciplinary care teams to implement individualized care plans
- Manage wound care, provide medication administration and education, and monitor disease progression
- Communicate updates and changes in patient condition to the clinical team promptly
Qualifications:
- Active RN license
- At least one year of recent home health RN experience
- Required experience with OASIS assessments and documentation
- Proficiency in HomeCare HomeBase (strongly preferred)
- Consistent ability to manage a structured, full-time visit schedule
- Reliable transportation and a valid drivers license
