A rewarding opportunity is available for an experienced Registered Nurse to provide home health services as part of a full-time, 13-week contract. With responsibility for delivering high-quality nursing care in a field-based setting, this position offers autonomy and meaningful patient interaction throughout the assigned territory.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Caseload: 30 points per week (typically 35 patient visits per day)
• Field-based with daily travel to patient homes (mileage reimbursement provided)

Key Responsibilities:

• Conduct thorough OASIS assessments and develop personalized care plans.
• Deliver a range of skilled nursing services, including wound care, IV therapy, medication administration, and management of chronic conditions.
• Monitor patient status and communicate any changes in condition to the care team and physicians.
• Educate patients and families on treatment, medications, ongoing care, and preventive strategies.
• Maintain detailed and timely documentation using Home Care Home Base (HCHB).
• Collaborate with interdisciplinary team membersincluding therapists and social workerspromoting cohesive patient care.
• Participate in case conferences, staff meetings, and ongoing education.
• Adhere to clinical and regulatory standards in all patient care and documentation.

Qualifications:

• Active RN License (in good standing) in the state of practice
• At least 1 year of recent Home Health experience (field experience preferred)
• Proficiency with Home Care Home Base (HCHB) and OASIS is required
• Reliable transportation, valid drivers license, and auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
