An exciting contract opportunity is available for an experienced Registered Nurse (RN) with a strong background in home health care. This role focuses on providing patient-centered care in home settings with an emphasis on professionalism, clinical expertise, and compassionate service. OASIS experience is required.
Key responsibilities include:
- Deliver comprehensive nursing care to home health patients.
- Conduct thorough patient assessments including vital signs, medication management, and wound care.
- Develop and implement customized care plans tailored to patient needs.
- Educate patients and families on disease management, treatment protocols, and preventive strategies.
- Collaborate effectively with physicians, therapists, and allied healthcare professionals to ensure optimal care.
- Maintain accurate, timely documentation of all care activities in electronic medical records (EMR).
- Observe and report any changes in condition to the supervising physician.
- Uphold state and federal regulations governing home health practices.
Key qualifications:
- Current RN license.
- At least three months of recent home health experience utilizing OASIS.
- HCHB (Homecare Homebase) experience strongly preferred.
- Strong foundation in IV therapy, wound care, and medication management.
- Proficiency with EMR systems.
- Excellent communication skills and a collaborative approach.
- Ability to work autonomously and manage caseload efficiently.
- Valid drivers license and dependable transportation.
