Key Responsibilities:
- Provide skilled nursing care and clinical interventions within patients homes.
- Conduct thorough patient assessments and complete OASIS documentation accurately and promptly.
- Collaborate closely with physicians and interdisciplinary care teams to implement personalized care plans.
- Manage wound care, medication administration, patient education, and chronic disease monitoring.
- Report patient condition updates and coordinate with the clinical team for ongoing care adjustments.
Qualifications and Experience:
- Current, active RN license.
- Minimum of one year recent experience in home health nursing.
- Proven expertise with OASIS documentation is required.
- Proficiency with HomeCare HomeBase software is strongly preferred.
- Ability to maintain a structured, full-time schedule including daily patient visits.
- Reliable transportation and a valid drivers license are essential.
