Enjoy a rewarding, field-based nursing role with the opportunity to build impactful patient relationships. This 13-week contract position offers full-time hours in a home health setting, delivering high-quality nursing care and supporting patient independence.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Position Details:

• Field-based; daily travel to patient homes within a designated territory
• Caseload of approximately 30 points per week (averaging 35 patient visits per day)
• Utilize Home Care Home Base (HCHB) for electronic documentation
• Complete OASIS assessments to guide patient-centered care planning

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing interventions, including wound care, medication management, IV therapy, and chronic disease support
• Monitor patient progress, flag changes in condition, and communicate with care teams and physicians
• Provide patient and family education on disease management, medication regimes, and preventive care
• Accurately document all care activities in HCHB
• Work collaboratively with physical/occupational therapists, social workers, and home health aides to coordinate comprehensive care
• Participate in case conferences, staff meetings, and ongoing education
• Adhere to all compliance, documentation, and care standards

Qualifications:

• Current, active RN license in the state of practice
• At least 1 year of recent field-based Home Health nursing experience
• Proficient in Home Care Home Base (HCHB) and experienced with OASIS
• Reliable vehicle, valid drivers license, and proof of auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
