An exciting opportunity awaits a skilled Registered Nurse with a background in home health to support patients through contract work. The ideal candidate will provide high-quality care in home settings, aiming to improve patient outcomes and promote overall well-being.
Key Responsibilities:
- Deliver comprehensive nursing care to patients in their homes
- Complete initial and ongoing patient assessments, including vital signs, medication management, and wound care
- Create and implement individualized care plans based on thorough assessments
- Educate patients and families on disease management, treatment protocols, and preventive health practices
- Collaborate closely with physicians, therapists, and multidisciplinary healthcare providers for seamless case management
- Accurately document all patient interactions using EMR systems
- Monitor and promptly report any change in patient status to supervising providers
- Ensure all practices adhere to state and federal home health care regulations
Schedule/Hours:
- Contract position (exact duration to be discussed)
Qualifications:
- Active RN license
- At least 3 months of home health experience with hands-on OASIS documentation is required
- Experience with HCHB (Homecare Homebase) strongly preferred
- Advanced clinical skills in IV therapy, wound care, and medication administration
- Proficient with electronic medical records systems
- Exceptional communication and interpersonal abilities
- Capable of working independently and managing multiple priorities
- Valid drivers license and dependable transportation
