Job Description

Advance your nursing career with this rewarding opportunity delivering home health services in a field-based role. As a contract RN, youll support patients in their homes, ensuring they receive exceptional, personalized care throughout a 13-week assignment.

Schedule & Setting:

  • Full-time contract, 13 weeks
  • Monday Friday, 8:00 AM 5:00 PM
  • Daily travel within your assigned territory (mileage reimbursement provided)
  • Typical caseload: 35 patient visits per day (30 points per week)
  • Field-based with electronic documentation

Qualifications:

  • Active, unrestricted RN license for the state
  • At least 1 year of recent home health nursing experience required (field experience highly valued)
  • Proficiency in Home Care Home Base (HCHB) electronic documentation system
  • Familiarity and experience with OASIS assessments
  • Reliable vehicle, valid drivers license, and current auto insurance

Key Responsibilities:

  • Complete comprehensive OASIS assessments and design individualized care plans
  • Deliver direct skilled nursing interventions, including wound care, medication administration, IV therapy, and chronic disease management
  • Monitor patient progress and communicate significant status changes to physicians and care teams
  • Provide clear education to patients and family members regarding treatment plans, medications, and preventive care
  • Accurately document all visits and interventions using HCHB
  • Engage in interdisciplinary collaboration with other healthcare professionals (therapists, social workers, aides)
  • Participate in team meetings, case conferences, and ongoing educational sessions
  • Ensure compliance with all regulatory standards and agency procedures

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
