Job Description

A dynamic nursing opportunity awaits experienced Registered Nurses specializing in home health care. This full-time, 13-week contract position centers on field-based services, providing essential skilled nursing directly to patients within their homes. The role follows a Monday through Friday schedule, 8:00 AM to 5:00 PM, balancing a manageable caseload (35 patient visits per day, averaging 30 points weekly).

Key Responsibilities:

  • Conduct thorough OASIS assessments and design personalized care plans.
  • Deliver skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, identify status changes, and promptly update interdisciplinary care teams and physicians.
  • Educate patients and families on disease management, treatments, medications, and preventive strategies.
  • Maintain precise, timely documentation utilizing the Home Care Home Base (HCHB) electronic system.
  • Collaborate with fellow cliniciansphysical therapists, occupational therapists, social workers, and home health aidesto ensure comprehensive care.
  • Engage in interdisciplinary case conferences, team meetings, and ongoing professional development.
  • Uphold standards and regulatory guidelines in documentation and care delivery.

Qualifications:

  • Active, unencumbered RN license in the state of practice.
  • At least one year of recent home health nursing experience (field experience strongly preferred).
  • Demonstrated proficiency in Home Care Home Base (HCHB) and OASIS assessment.
  • Reliable transportation, a current drivers license, and proof of auto insurance.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
