Your next nursing opportunity starts here. We are seeking an experienced Registered Nurse (RN) skilled in home health care and OASIS documentation for a full-time, 13-week contract assignment. This position offers the chance to deliver patient-centered care while working within a supportive, professional team environment.
Schedule & Hours:
- 13-week contract, potential for extension
- Full-time, Monday through Friday
- On-call responsibilities may be required
- Structured daily visit schedule
Key Responsibilities:
- Deliver skilled nursing interventions in the home setting, ensuring patients receive quality care
- Conduct comprehensive patient assessments and accurately complete OASIS documentation in a timely manner
- Collaborate closely with physicians and care teams to create and maintain individualized care plans
- Provide wound care, medication administration, disease monitoring, and patient education
- Relay important changes or updates in patient condition to the clinical team for coordinated care
Qualifications:
- Active RN license
- Minimum of one year recent home health RN experience
- Demonstrated proficiency with OASIS documentation (required)
- Experience with HomeCare HomeBase (strongly preferred)
- Ability to independently manage a structured, full-time visit schedule
- Reliable transportation and a valid driver's license
