Job Description

An experienced Home Health Registered Nurse (RN) is needed to deliver skilled nursing care in patient homes, focusing on quality, continuity, and patient-centered practices. This role suits a self-motivated, organized professional who excels in independent work, clinical excellence, and clear patient communication.

Key Responsibilities:

  • Perform comprehensive OASIS assessments and develop personalized care plans.
  • Provide nursing care including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, report changes to care teams and physicians promptly.
  • Educate patients and families on health conditions, medications, and preventive care.
  • Maintain precise and timely documentation using Home Care Home Base (HCHB).
  • Coordinate with therapists, social workers, and aides to ensure cohesive care delivery.
  • Attend case conferences, staff meetings, and continue professional education.
  • Follow agency and regulatory standards for documentation and patient care.

Qualifications and Experience:

  • Active RN license in good standing.
  • Minimum one year of recent home health nursing experience (field experience preferred).
  • Proficiency with Home Care Home Base (HCHB) and OASIS assessments.
  • Strong clinical and critical thinking skills.
  • Valid drivers license, reliable transportation, and auto insurance.
  • Ability to manage a flexible, field-based schedule independently.
  • Excellent communication and organizational capabilities.
  • BSN preferred.
  • Experience handling 2532 points weekly.
  • Knowledge of Medicare and home health compliance is a plus.

Schedule and Contract Details:

  • Full-time, 13-week contract.
  • Monday through Friday, 8:00 AM to 5:00 PM.
  • Caseload of 2532 points per week, typically 35 patient visits daily.
  • Field-based role with daily travel to patient homes; mileage reimbursed.
