This role presents a unique environment in which seasoned nurses can refine their clinical acumen, learn from leading professionals, and enhance their ability to manage complex cases across home health and hospice. Working closely with patients managing chronic conditions and end-of-life care, youll develop a deeper skill set and make a lasting impact on individuals and families alike.
Schedule/Hours:
- 13-week assignment
- 40 guaranteed hours per week (except during holidays)
- 10-hour day shifts, 8:00 am to 6:00 pm
- Rotating schedules: ThursSun or SatTues
- No on-call or call back required
Qualifications:
- WI RN License, BLS (American Heart), and valid drivers license (must be submitted with application)
- Minimum 1-2 years recent Home Health and Hospice experience
- Recent Oasis and Epic charting experience required
- Comfortable with 1:1 adult patient care, chronic disease management, hospice admissions, death visits, and symptom management
- Physically capable of climbing stairs, driving extensive distances, and entering variable home environments
Role Responsibilities:
- Deliver skilled nursing care to adults with chronic or terminal illnesses in their homes, including wound care and IV administration
- Timely and thorough documentation using Epic and Oasis
- Flexibly adapt to changing schedules and conditions
- Prioritize time management and maintain resilience in a dynamic setting
- Provide compassionate care in diverse and sometimes challenging home environments
