Deliver essential care to patients in the comfort of their own homes with a rewarding field-based role as a Registered Nurse specializing in Home Health. This full-time, 13-week contract offers a structured weekday schedule: Monday through Friday, 8:00 AM 5:00 PM, providing you with work-life balance as you support an assigned caseload of approximately 3 to 5 patient visits per day. Daily travel within your territory is an integral part of this position, with mileage reimbursement provided.

Required Qualifications:

• Active RN license (in good standing) in the state of practice
• At least 1 year of recent experience in Home Health, with field experience preferred
• Hands-on knowledge of Home Care Home Base (HCHB)
• Demonstrated proficiency in OASIS assessments
• Reliable transportation, valid drivers license, and current auto insurance

In this position, youll:

• Conduct thorough assessments using OASIS to develop individualized care plans
• Deliver skilled nursing services, such as wound care, medication administration, IV therapy, and ongoing chronic disease support
• Monitor patient progress, identify clinical changes, and ensure seamless communication with the interdisciplinary care team
• Educate patients and families on condition management and preventive health practices
• Accurately document all care in Home Care Home Base (HCHB)
• Participate in collaborative case conferences and team meetings
• Uphold regulatory and agency standards for patient care and documentation

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
