A dedicated Registered Nurse is needed for a 13-week full-time home health contract, delivering in-home care and making a real difference in patients lives. This role is ideal for RNs who are confident in their clinical skills, possess strong documentation ability (especially with OASIS), and thrive on managing a set schedule.
Schedule:
- Full-time, Monday through Friday
- On-call may be required
Key Qualifications:
- Active RN license
- Minimum of 1 year of recent experience as a home health RN
- OASIS documentation proficiency is required
- Familiarity with HomeCare HomeBase is strongly preferred
- Reliable transportation and a valid drivers license
- Comfortable maintaining a structured schedule with regular daily visits
Primary Responsibilities:
- Provide comprehensive in-home nursing care and skilled interventions
- Perform thorough patient assessments and complete accurate OASIS documentation on time
- Collaborate closely with physicians and interdisciplinary care teams to develop and deliver tailored care plans
- Carry out wound care, medication management, patient education, and chronic disease monitoring
- Communicate patient status and any clinical changes to team members
