An opportunity is available for a Registered Nurse (RN) with home health expertise to provide high-quality patient care on a contract basis. This role requires a nurse skilled in comprehensive assessments, care planning, and patient education within the home health setting. Experience with OASIS documentation is essential to succeed in this position.

Key Responsibilities:

• Deliver thorough nursing care to patients in their residences.
• Conduct initial and ongoing patient assessments, including vital signs, medication administration, and wound care management.
• Create and implement personalized care plans tailored to patient needs.
• Educate patients and family members on managing diseases, treatment adherence, and preventive measures.
• Collaborate with a multidisciplinary team to coordinate effective patient care.
• Accurately document all care activities in electronic medical records.
• Monitor and communicate any changes in patient conditions.
• Ensure adherence to all applicable home health care regulations.

Qualifications:

• Active Registered Nurse license.
• At least three months of home health experience with proficiency in OASIS.
• Experience with Homecare Homebase (HCHB) is highly advantageous.
• Strong clinical skills, including intravenous therapy, wound care, and medication management.
• Comfortable using electronic medical records systems.
• Excellent communication abilities and interpersonal skills.
• Self-motivated with strong time management capabilities.
• Valid drivers license and reliable transportation required.
