Job Description

An experienced Registered Nurse (RN) with a strong background in home health nursing is sought for a contract position in Waterbury, CT. Key Responsibilities:

  • Deliver comprehensive nursing care to patients within their homes.

  • Educate patients and family members on disease management, treatment options, and preventive health.

  • Monitor patient progress and report any significant changes to supervising physicians.

Qualifications:

  • Active Registered Nurse (RN) license.
  • Minimum of three months of experience in home health nursing with proficiency in OASIS.
  • Experience with Home Care Home Base (HCHB) software is highly desirable.
  • Strong clinical competencies, including IV therapy, wound care, and medication management.
  • Skilled in electronic medical record systems.
  • Excellent communication and interpersonal skills.
  • Ability to work independently while managing time effectively.
  • Valid drivers license and reliable transportation for home visits.
