We are seeking a dedicated Registered Nurse with strong OASIS experience for a 13-week full-time contract focusing on patients in their homes. This position is ideal for nurses who excel at managing schedules, maintaining documentation accuracy, and making meaningful connections with patients and their families.
Schedule:
- 13-week contract (potential for extension)
- Monday through Friday, full-time hours
- On-call rotations may be required
Key Responsibilities:
- Deliver comprehensive in-home nursing care and skilled clinical interventions
- Complete patient assessments, ensuring timely, accurate OASIS documentation
- Collaborate with physicians and interdisciplinary care teams to create and implement individualized care plans
- Provide wound care, medication administration, disease monitoring, and patient/caregiver education
- Communicate promptly with the clinical team regarding patient status and updates
Desired Qualifications:
- Active RN license required
- At least one year of recent experience as a home health RN
- OASIS documentation proficiency is mandatory
- Familiarity with HomeCare HomeBase preferred
- Confident in adhering to structured schedules and daily patient visit expectations
- Reliable transportation and a valid drivers license
