Your next nursing opportunity awaits as a Registered Nurse specializing in home health care. This role is perfectly suited for experienced RNs with strong OASIS documentation skills and a passion for delivering patient-centered care in a home setting. Youll be making a significant impact while managing a full-time schedule with daily in-home visits for a 13-week assignment.
Schedule & Contract Details:
- Full-time, Monday through Friday
- On-call responsibilities may be required
- 13-week contract duration, with potential for extension
What You'll Do:
- Deliver skilled nursing care and specialized interventions in patients homes
- Conduct thorough patient assessments, ensuring timely and accurate OASIS documentation
- Collaborate with multidisciplinary teams to create and update individualized care plans
- Provide wound care, manage medications, educate patients/families, and monitor chronic disease conditions
- Communicate condition updates swiftly to the clinical care team
Role Requirements:
- Active, unrestricted RN license
- At least one year of recent home health RN experience
- Strong working knowledge of OASIS is a must
- HomeCare HomeBase proficiency is highly preferred
- Capable of independently managing a structured patient visit schedule
- Valid drivers license and dependable transportation
