Your next nursing opportunity starts here. We are seeking a committed Registered Nurse with strong OASIS expertise for a 13-week, full-time home health assignment. This position is designed for a confident clinician skilled in both patient care and meticulous documentation, ready to make a meaningful difference in the lives of patients in their own homes.
Schedule:
- Full-time, Monday through Friday
- On-call duties may be required
- Structured daily visit expectations
Key Responsibilities:
- Deliver skilled in-home nursing care and evidence-based interventions
- Perform comprehensive patient assessments and complete OASIS documentation accurately and promptly
- Coordinate with physicians and multidisciplinary teams to develop and implement personalized care plans
- Carry out wound care, medication administration, disease management, and patient/family education
- Monitor and communicate any changes in patient condition to the care team
Preferred Qualifications:
- Active RN license (in good standing)
- Minimum of 1 year of recent home health RN experience
- Proficient in OASIS documentation
- Experience with HomeCare HomeBase is strongly preferred
- Proven ability to manage a full-time caseload and adhere to set schedules
- Valid drivers license and reliable transportation
