Job Description
Deliver compassionate care and make a lasting impact in the community by providing direct patient services in a field-based home health setting. As an experienced RN, you will be responsible for coordinating and delivering skilled nursing interventions to patients in their homes, ensuring quality outcomes and continuity of care throughout a full-time, 13-week contract assignment.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Qualifications:
- Active and unrestricted RN license for the state of practice
- At least 1 year of recent hands-on home health nursing experience (field roles strongly preferred)
- Solid proficiency with Home Care Home Base (HCHB) electronic medical records system
- Demonstrated OASIS assessment experience
- Dependable vehicle, valid drivers license, and current auto insurance
Primary Responsibilities:
- Complete thorough OASIS assessments and design customized care plans to support patient goals
- Deliver comprehensive skilled nursing care such as wound management, IV therapy, medication administration, and chronic disease management
- Monitor patient status, identify and report changes, and collaborate closely with physicians and the care team
- Provide patient and family education on disease processes, medications, treatment plans, and preventive health
- Maintain meticulous, timely documentation using Home Care Home Base (HCHB)
- Coordinate with multidisciplinary providersincluding PT, OT, social workers, and HHAsto ensure high-quality care delivery
- Attend regular staff meetings, case conferences, and ongoing professional development
- Consistently adhere to regulatory and organizational standards in all aspects of care

$1,790 - $1,958/Week Estimated weekly earnings represent hourly wages, based on 36 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
