Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be required
Qualifications:
- Active RN license
- At least one year of recent home health nursing experience
- Proficient with OASIS assessments and documentation
- Experience with HomeCare HomeBase (strongly preferred)
- Reliable transportation along with a valid drivers license
- Organized, detail-oriented, and confident managing a daily visit schedule
Position Responsibilities:
- Deliver high-quality skilled nursing to patients in their homes
- Complete patient assessments and OASIS documentation promptly and accurately
- Collaborate with physicians and care teams to implement individualized care plans
- Administer wound care, monitor disease progression, manage medications, and provide patient education
- Communicate status updates and any changes in patient conditions to the clinical team
- Maintain accurate visit and care records
