This is a full-time, 13-week contract opportunity designed for Registered Nurses who excel in patient-centered care and thrive in an autonomous work environment. Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Current, active RN license (state of practice, in good standing)
• At least 1 year of recent home health nursing experience (field experience strongly preferred)
• Proficient in Home Care Home Base (HCHB) electronic documentation
• Prior OASIS assessment experience
• Reliable vehicle, valid drivers license, and current auto insurance

Primary Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management
• Monitor patient conditions, update care plans, and promptly communicate with clinical teams and physicians
• Educate patients and families on disease management, medications, and preventive measures
• Accurately document all care activities in Home Care Home Base (HCHB)
• Participate in case conferences, staff meetings, and ongoing training as needed
• Collaborate with therapists, social workers, and home health aides to ensure integrated, high-quality care
• Adhere to regulatory and agency standards for documentation and care delivery

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
