Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Perform thorough OASIS assessments and develop comprehensive, individualized care plans.
• Deliver skilled nursing care, such as wound management, medication administration, IV therapy, and support for chronic conditions.
• Monitor patient outcomes, identify health status changes, and communicate rapidly with care teams and physicians as needed.
• Educate patients and families on disease management, medications, and preventative health practices.
• Maintain precise documentation using Home Care Home Base (HCHB).
• Collaborate closely with physical and occupational therapists, social workers, and aides to ensure seamless patient care.
• Attend case conferences, staff meetings, and pursue continuing education.
• Uphold regulatory standards and best practices for home health care.

Position Requirements:

• Active and unencumbered RN license for the state of practice
• At least one year of recent home health nursing experience, preferably with direct fieldwork
• Proficiency with Home Care Home Base (HCHB) and OASIS required
• Reliable transportation, current drivers license, and valid auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
