Advance your nursing career with a dynamic role providing home health services throughout the Wallingford area. This full-time, 13-week contract position centers on delivering high-quality care directly to patients in their homes, with a focus on exceptional clinical practice and patient education.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing care such as wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient progress, promptly communicate any status changes to the care team and physicians.
• Educate patients and their families on health management, treatments, and disease processes.
• Maintain detailed, timely electronic documentation using Home Care Home Base (HCHB).
• Coordinate care with physical therapy, occupational therapy, social work, and home health aide colleagues.
• Participate in staff meetings, case conferences, and continuing education sessions.
• Ensure all care and documentation meet agency and regulatory standards.
• Travel daily to patient homes within your assigned territory, with mileage reimbursement provided.

Requirements:

• Active RN license in good standing for the state of practice.
• At least 1 year of recent home health nursing experience; field work strongly preferred.
• Direct experience with OASIS assessments.
• Proficiency in Home Care Home Base (HCHB) documentation.
• Reliable transportation, valid drivers license, and proof of auto insurance.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
