Schedule & Hours:
- Full-time, Monday through Friday
- On-call availability may be required
- Daily visit targets with a consistent routine
Qualifications:
- Active RN license
- At least 1 year of recent home health RN experience
- Proficiency in OASIS documentation
- Familiarity with HomeCare HomeBase (strongly preferred)
- Valid drivers license and reliable transportation
Responsibilities:
- Deliver skilled home nursing care and interventions in patient residences
- Conduct comprehensive assessments and complete timely OASIS documentation
- Develop and collaborate on individualized patient care plans
- Perform wound care, medication administration, and chronic disease management
- Educate patients and families on care processes and health maintenance
- Communicate status changes and updates to the clinical team promptly
