This is a 13-week assignment with the potential for extension, offering a structured Monday through Friday schedule, with possible on-call responsibilities.

Schedule & Hours:

• Full-time, Monday through Friday
• On-call probable

Key Responsibilities:

• Deliver high-quality in-home nursing care and skilled interventions
• Conduct patient assessments and complete OASIS documentation thoroughly and on schedule
• Collaborate closely with physicians and interdisciplinary care teams to create and execute individualized care plans
• Provide wound care, medication administration, patient and family education, and ongoing disease monitoring
• Maintain consistent communication regarding patient conditions and care updates

Desired Qualifications:

• Active RN license required
• At least one year of recent experience in home health nursing
• Proven OASIS assessment and documentation proficiency
• Experience with HomeCare HomeBase is strongly preferred
• Ability to manage a structured schedule and daily patient visits
• Valid drivers license and reliable transportation

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
